Although your application has been submitted, we still need both a sponsorship form from the pastor of your parish. We will only contact you if we have received your renewal application and the sponsor form from your pastor. Please follow the instructions below.

1. Download the form below and send to your pastor, or share the link to this page with your pastor.
2. Once your pastor completes the form, they will download the form and send it to John Cammarata at [email protected].
3. Once we recieve this form, you will be notified about next steps in the process.
